Choosing an SPF lip balm, and why an ordinary balm offers no sun protection

SPF is a tested number, not an ingredient, and nobody applies the 2 mg per square centimetre used to measure it. Reapply, and check UVA protection too.

The number on the front of an SPF lip balm is a laboratory result obtained under conditions you will never reproduce, on a part of the body that is licked, wiped and eaten off all day. That does not make it worthless. It makes it a figure you have to read properly, apply generously and refresh often. Short answer

Choose a broad spectrum product labelled SPF 30 or higher. Apply enough to leave a visible film on both lips, roughly 10 mg, not a quick swipe, and reapply every two hours outdoors and after every meal, drink or towel. SPF 30 blocks about 96.7 percent of erythemal ultraviolet at the 2 mg per square centimetre test dose, and considerably less at real application rates.

What the number on the front actually measures

SPF is the ratio between the ultraviolet dose needed to redden protected skin and the dose needed to redden the same person's unprotected skin. It is measured in vivo on human volunteers under a solar simulator, on the back rather than the lips, with the product spread at exactly 2 milligrams per square centimetre. The method is ISO 24444 internationally and 21 CFR 201.327 in the United States, and the two are close enough that the number travels between markets.

Two consequences follow, and both are routinely misread. The first is that SPF describes protection against erythema, meaning sunburn, which is overwhelmingly a UVB effect. It is not a general index of sun damage and it says almost nothing on its own about UVA. The second is that the scale is a ratio, not a percentage, and it flattens quickly. The fraction of erythemally weighted ultraviolet that gets through is simply one divided by the SPF.

Transmitted erythemal ultraviolet is 1/SPF by definition. The reduced-dose columns use the exponential model of Faurschou and Wulf (British Journal of Dermatology, 2007), in which effective protection is approximately SPF raised to the power of the applied fraction of 2 mg per square centimetre. Those figures are modelled from body-site sunscreen data, not measured on lips, and should be read as an order of magnitude.

Read across the top row and the practical argument for buying a high number appears. The difference between SPF 15 and SPF 50 at full dose is small in absolute terms, 6.7 percent transmitted against 2.0 percent. The difference at the dose people actually use is not small at all, because the shortfall bites hardest at the bottom of the scale. Buying SPF 30 or 50 is best understood as buying headroom against your own application habits rather than buying a large increment of protection at the test dose.

Why lips are the wrong tissue to skimp on

The vermilion, the red part of the lip, is thin, poorly keratinised, and carries very little melanin compared with the skin around it. It has essentially no sebaceous or sweat glands, so it has neither the surface film nor the pigment that protect facial skin. That is the same anatomy that makes lips prone to cracking in dry air, discussed under cold weather skin, and it makes them poor at handling ultraviolet for exactly the same reason.

Geometry compounds it. At the middle of the day the sun is high, and the lower lip presents a surface tilted towards it, while the upper lip is shaded by the nose and by the overhang of the lip itself. The result shows up clearly in clinical series: actinic cheilitis, the chronic sun-damage change of the lip, is overwhelmingly a lower lip disease, and so is squamous cell carcinoma at that site. A 25 year Brazilian hospital series reported the lower lip as the site of 70.6 percent of lip squamous cell carcinomas.

Actinic cheilitis is classified as a potentially malignant disorder rather than a cosmetic nuisance. It looks like chapping that never fully resolves: persistent dryness and scaling, a blurred boundary where the vermilion meets the skin, pale atrophic areas, sometimes a thickened white patch. Because it mimics ordinary chapping, people treat it with more balm for months. If a patch on your lower lip has not settled in three weeks of bland care, that is the point at which the answer stops being a product.

Get it looked at

A rough, scaly or white patch on the lower lip that persists beyond about three weeks, a spot that bleeds or crusts and re-forms, an ulcer that does not heal, or a loss of the sharp line between lip and skin all need a clinician rather than another balm. The dose gap, and why it is worse on lips

Consumer application studies of body and facial sunscreen consistently find people applying somewhere between a quarter and a half of the 2 mg per square centimetre test dose. No equivalent measurement exists for lip products, which is worth saying plainly rather than inventing one, but every mechanism that causes under-application elsewhere is stronger here.

Take the vermilion of both lips as roughly 5 square centimetres, an approximation rather than a measurement, and the test dose corresponds to about 10 milligrams per application. That is a real amount of product: enough to see and to feel, not a single quick pass with a stick. Then the film starts leaving. Drinking removes a lip product against the rim of a glass, eating removes it mechanically, speaking and licking abrade it, and a towel takes off whatever is left. A face sunscreen sits undisturbed for hours; a lip product rarely survives one coffee. This is why reapplication frequency, not the printed number, is the variable that determines what protection you actually receive over a day outdoors.

UVA is a separate claim, and it has its own mark

A product can post a high SPF and still let through most of the UVA, because SPF is calibrated on erythema. Every major market therefore added a second signal, and they are not the same signal.

The single most useful habit is to treat the UVA mark as a modifier of the SPF, not as an independent score. A high SPF with the appropriate regional UVA mark is the combination worth looking for. Where a lip product carries an SPF and no UVA indication at all, assume the UVA performance is unknown. A drug in one market, a cosmetic in another

The same tube can be two different categories of product depending on where it is sold, and this changes what the manufacturer had to prove.

In the United States, a product that claims sun protection is an over-the-counter drug regulated under the sunscreen monograph rather than a cosmetic. The pack must carry a Drug Facts panel, the active filters with their percentages, and the SPF stated in the monograph format. Only filters in the monograph may be used, which is why the American filter list is shorter and older than the European one.

In the European Union and the United Kingdom, sunscreen is a cosmetic under Regulation (EC) No 1223/2009 and its retained British equivalent, with permitted filters listed in Annex VI. Efficacy is steered by Commission Recommendation 2006/647/EC, which is a recommendation rather than binding law but is followed almost universally: it sets the one third UVA rule, restricts labelling to the categories low, medium, high and very high with a fixed set of SPF numbers, and caps the printed value at 50+. Australia treats primary sunscreens as therapeutic goods under the Therapeutic Goods Administration, tested to AS/NZS 2604, with lip products falling either side of the line depending on the claim made.

For a buyer this mostly matters as a reading skill: an American pack that lists actives under Drug Facts is telling you the same things a European pack tells you under Ingredients, in a different order. For anyone selling, the line between the two categories is the whole subject of cosmetic versus drug claims, and it is not a line to approach casually.

How much to apply, and how often

  1. Cover the whole vermilion and slightly past it. Sun damage does not stop at the wet line. Take the product out over the border onto the skin above the upper lip and below the lower lip, which is where actinic change often begins.
  2. Apply until you can see it. Two or three passes with a stick, or a deliberate fingertip layer from a pot. A film you cannot see is a film that is nowhere near 2 milligrams per square centimetre.
  3. Let it set before you drink. A minute or two of not pressing your lips together or picking up a glass keeps more of the film in place.
  4. Reapply every two hours outdoors, and after every eating, drinking, swimming or towelling event. On lips these events, not the clock, are usually what triggers reapplication. Assume it has gone after a meal.
  5. Do not rely on the balm alone. A brimmed hat changes the geometry that puts the lower lip in the firing line at midday, and it does not wash off in a drink.
  6. Keep the tube where you will use it. Cabin bags and hot cars both matter for lip products, as set out in flying and travelling with balms.
Try this

Weigh the tube on a jeweller's scale at the start of a sunny day out and again that evening. If a full day outdoors has used less than a gram, you were not applying anything like the tested dose. It is a cruder version of the same measurement makers use when they work out cost per application in lip balm price per gram, and it settles the argument quickly.

When the filters themselves are the problem

Ultraviolet filters are among the more common causes of photoallergic contact dermatitis, in which a molecule becomes an allergen only after light has altered it. The European multicentre photopatch test study found organic filters, particularly benzophenone-3 and octocrylene, among the most frequently identified photoallergens, alongside topical ketoprofen. On lips this presents as burning, swelling, scaling or persistent chapping that gets worse in sun, which is easily misread as needing more balm. Keep one unfiltered balm for indoor and overnight use so your lips are not carrying filters twenty four hours a day for no benefit, and if you suspect a reaction, work through it methodically using how to patch test a balm before buying a fourth product.

Mineral filters are the usual fallback. Zinc oxide and titanium dioxide are not photoallergens and are the sensible choice for anyone with a filter reaction, for children, and for very reactive lips; the trade-off is a whiter cast and a heavier feel. The gloss myth, and what the physics supports

The claim circulates that a shiny lip gloss concentrates ultraviolet like a lens and is therefore worse than wearing nothing. The lens part is wrong. A thin film sitting on a curved lip does not focus light onto the tissue underneath it in any meaningful way, and no clinical evidence links gloss wearing to lip cancer.

What is physically defensible is smaller and duller. A dry, rough, slightly scaly lip surface scatters and reflects a fraction of incident light away. Wetting that surface with a clear oil or gloss smooths it optically and reduces that surface reflectance, so slightly more of the incident ultraviolet enters the tissue instead of bouncing off. The effect is modest, it is a change of a few percent in reflectance rather than an amplification, and the optical behaviour of these films is set out in lip colour and gloss physics.

So the honest version is: an unfiltered gloss gives you no protection and may marginally reduce the small amount of natural surface reflection you had. That is a reason to wear a filtered product outdoors, not a reason to fear gloss. Opaque pigmented products are a different case, since iron oxides and titanium dioxide in a tinted stick do block some ultraviolet, but the amount is untested and unlabelled and should not be counted on. The decision rule, and what SPF does not cover

Outdoors, buy broad spectrum SPF 30 or more with the UVA mark used in your market, apply it visibly, and reapply after every meal, drink and swim. Indoors and overnight, use a plain unfiltered balm chosen for barrier repair rather than protection, on the reasoning set out in ingredients for chapped lips. If a rough patch on the lower lip persists beyond three weeks, stop buying products and see a clinician.

Three limits are worth being explicit about. An SPF lip balm is protection against erythema and, if it carries the right mark, partial protection against UVA; it is not protection against everything the sun does, and it does not undo damage already accumulated over decades. The number was earned at a dose you will not match, so treat SPF 30 as the floor rather than the target. And no filter addresses the other lip problems people buy these products for: wind, dry air, licking, and the mechanical damage of cold. Those want a different product, applied at different times, and there is no shame in owning two. What SPF should a lip balm be?

How often should I reapply SPF lip balm?

Every two hours outdoors, and immediately after eating, drinking, swimming or towelling your face. Lip products come off faster than anything else you apply, because a glass rim, food and your own tongue remove the film mechanically. On lips the trigger for reapplication is usually an event rather than the clock, so assume the product has gone after a meal.

Does an ordinary lip balm without SPF protect against the sun?

No. Wax and oil do not absorb ultraviolet in any useful amount, and there is no tested protection factor for a plain balm. An unfiltered balm is for barrier care: it reduces water loss and protects against wind and cold. If a lip product does not carry a tested SPF on the pack, treat it as giving no sun protection at all.

What does the UVA in a circle symbol mean?

It is the European mark, also used in the United Kingdom, indicating that the product's UVA protection factor is at least one third of its labelled SPF and that its critical wavelength is at least 370 nanometres. Because it is a ratio, the mark on an SPF 15 product represents much less absolute UVA protection than the identical mark on an SPF 50 product.

Can I make my own SPF lip balm at home?

No. SPF is a measured result from a standardised test on a finished formulation, not something conferred by adding zinc oxide to a melt, and the dispersion, film continuity and settling in a home batch are all uncontrolled. Is lip gloss bad for sun exposure?

Not in the way the myth says. A gloss film does not act as a lens focusing ultraviolet onto the lip. What it does is smooth a rough dry surface, slightly reducing the light that reflects away, so marginally more enters the tissue. The effect is small. The real issue is simply that an unfiltered gloss provides no protection at all.
