Balm faults are unusually diagnosable. There is no microbiology to guess at and no pH to chase, so a fault almost always comes down to one of four things: the wax percentage, the crystallisation of a hard fat, the cooling rate, or water where it should not be.

Texture faults
The batch set, but it does not feel right.

Setting and filling faults

Colour, scent and taste
Faults you can see or smell, which are usually solubility or oxidation in disguise.
