Balm vs salve vs ointment vs cream: what the words actually mean

One comparison table across balm, salve, ointment, butter, cream, lotion, oil and gel, plus honest notes on where the words overlap and which to pick.

These five words describe overlapping things, and most of the disagreement about them is regional habit rather than chemistry. Two of them (ointment, cream) have real technical definitions behind them. Two of them (balm, salve) have none at all. Sorting out which is which makes shopping and formulating considerably easier, so this page draws the lines where they actually exist and says plainly where they do not.

Short answer

The only hard difference is water. Balms, salves and ointments are anhydrous, so they need no preservative and sit on the skin as a film. Creams and lotions are emulsions of water in oil and need both an emulsifier and a preservative. Balm and salve are informal texture words; ointment is a pharmaceutical dosage form.

Every other distinction in this vocabulary is convention. Thickness, packaging and the words on the front of the tin all vary between traditions and between markets, but the presence or absence of a water phase decides the formulation chemistry, the preservation question and, in some markets, the regulatory category.

The one line that is real: water content

Every meaningful property in this comparison falls out of a single question. Does the product contain water?

If it does, it needs an emulsifier to keep the two phases together, a preservative to stop things growing in the water, a pH to worry about, and a manufacturing process with two heated phases and a homogenisation step. If it does not, it needs none of those, and the failure modes change from microbial to physical. That is a real, sharp, structural division. Everything else in this vocabulary is a matter of degree and habit. Anhydrous formulation covers what that division costs and what it buys.

Occlusivity here is a property of the finished film, not of the format label. A balm built on jojoba and squalane is markedly less occlusive than one built on lanolin and beeswax, and a petrolatum ointment beats both. Composition wins over category every time.

Balm vs salve

There is no chemical distinction. Both are anhydrous, both are wax-set, both belong to the structure described in what is a balm. The useful distinction is conventional: salves are the softer, oilier, herbal end of the range and balms are the firmer, more buttery, more cosmetic end.

A classic salve is close to the old 1 part wax to 8 parts oil rule, which lands around 11% wax, with the oil usually infused with dried plant material and almost no butter in the formula. It is scooped, not swiped, and it is expected to feel slick rather than rich. A balm at the same wax level typically has 20-30% butter displacing liquid oil, which makes it thicker under the finger and less prone to running. Regionally, "salve" is a live everyday word in American English and reads as slightly archaic or medical in British English, where "balm" or simply "ointment" covers the same ground. That is the whole of the difference, and if a brand tells you their salve is a fundamentally different technology from their balm, they are describing a marketing decision.

Salve vs ointment

This is the pair where the words genuinely diverge, because one of them has an institution behind it. "Ointment" is a pharmaceutical dosage form: a semi-solid preparation intended for external application, conventionally built on a hydrocarbon base such as white petrolatum, sometimes with paraffin or a wax added to adjust consistency. When a pharmacopoeia says ointment, it means something specific about the base and the manufacturing standard.

The practical consequences are three. Ointments are usually medicated, so they carry an active ingredient and a regulated label. They are usually built on petrolatum or mineral oil rather than plant fats, which makes them more occlusive and far more oxidatively stable but also greasier and less pleasant. And they are held to pharmaceutical manufacturing and testing standards that a small-batch salve is not.

Where the words blur: plenty of unmedicated products are sold as "ointment" purely for the connotation of seriousness, and plenty of genuinely functional occlusive products are sold as "salve" because it sounds gentler. In the US, an anhydrous product making a skin protectant claim ("protects and helps relieve chapped or cracked skin") is an over-the-counter drug under the 21 CFR 347 monograph regardless of which word is on the front. What you call it does not change what it legally is.

Balm vs cream

A cream is an emulsion. Oil droplets are dispersed in water (or water droplets in oil, in the less common water-in-oil type) and held there by an emulsifier that sits at the interface. Typical face creams run 55-75% water. That water is doing several jobs: it carries humectants and water-soluble actives, it evaporates on application to give the cool light feel people associate with a good moisturiser, and it makes the product spread easily and cheaply.

The trade-off is that the same water requires a preservative system, keeps the pH in play, and gives the product a genuine microbial shelf life to test and defend. It also means that when you apply 1 g of cream, roughly 0.3 g of material is left on your skin once the water has gone; apply 1 g of balm and 1 g stays. This is why balm doses are so much smaller, and why people who move from cream to balm usually over-apply for the first week.

Functionally, a cream both supplies water and slows its loss. A balm only does the second half. That is not a defect, it is a division of labour, and it is the reason the two layer well: damp skin or a humectant product first, balm over the top. The mechanics are in moisturisers and the skin barrier.

Balm vs lotion, oil and gel

A lotion is a cream with more water and less oil, thin enough to pour. It exists to cover large areas fast and disappear. Against a balm it wins comfortably on cosmetic elegance and loses comfortably on residence time: a lotion on cracked heels is gone before it has done anything.

A plain oil is the interesting comparison, because it is anhydrous like a balm but has no structurant at all. It spreads further, absorbs faster, feels lighter, and provides much less occlusion because the film it leaves is thin and mobile. Adding 8% wax to that same oil turns it into a salve and roughly transforms how long it stays where you put it.

A gel is at the far end: mostly water, thickened with a polymer, essentially zero occlusion. It is the correct format for delivering water-soluble actives to skin that does not want lipids, and the wrong format for anything to do with dryness on its own.

Which one to reach for

Format follows the problem. These are general skincare suggestions, not medical advice; anything painful, spreading, weeping or not improving deserves a pharmacist or doctor rather than a formulation decision.

Safety

Do not apply any balm, salve or ointment to broken, weeping, burnt or infected skin without medical advice. An occlusive layer over an infection traps it. Anhydrous products contain no preservative, so a tub touched with dirty fingers and then applied to open skin is a real, if small, contamination route.

Where the vocabulary is genuinely unreliable

Three specific traps are worth knowing about.

"Balm" on an emulsion. Several well-known products called cleansing balms, lip balms and barrier balms are in fact water-containing creams. The word has drifted to mean "thick and rich" rather than "anhydrous". Check the ingredient list for aqua before you assume.

"Butter" on a cream. Body butter frequently means a whipped emulsion, not whipped anhydrous fat. If it has water in it, it needs the preservative, and if it is genuinely anhydrous it does not. Same word, two different products.

"Natural ointment" with no petrolatum. Fine as a description, but the occlusive performance of a plant-wax product is not the same as a petrolatum base, and it is honest to say so. That comparison, with what the measurements do and do not show, is in occlusive, emollient and humectant.

The habit worth building is to stop reading the noun on the front and start reading the first five entries on the back. Water or no water, wax or no wax, emulsifier or none, preservative or none. Those four answers tell you what you are holding, and the preservative question alone will tell you whether the front label is being accurate.

What is the difference between a balm and a salve?

Practically, softness and tradition rather than chemistry. Both are anhydrous and wax-set. A salve is usually the oilier, softer, herb-infused end of the family at roughly 8-15% wax and 70-90% liquid oil, scooped from a tin. A balm usually carries more butter and sets firmer. No regulator distinguishes them and brands use both words freely.

Is an ointment stronger than a balm?

Not inherently. "Ointment" is a pharmaceutical dosage form, so ointments are usually medicated and usually built on petrolatum or paraffin, which is more occlusive than plant wax and oil. The word signals a regulated product with an active ingredient, not a more powerful base. An unmedicated ointment base and a balm do broadly the same physical job.

What is the difference between a balm and a cream?

Water. A cream is an emulsion, typically 55-75% water held in oil by an emulsifier, and it needs a preservative. A balm has no water at all, no emulsifier and normally no preservative. A cream feels light and cool because water evaporates from it. A balm leaves everything you applied on the skin.

What is the difference between a balm and a lotion?

The same difference as with a cream, only more so. A lotion is a thinner emulsion, often 70-85% water, designed to spread quickly over large areas and sink in. A balm is 100% lipid and sits on the surface. A lotion delivers water-soluble ingredients; a balm cannot without an added solubiliser.

Is body butter a balm?

Only loosely. Whipped body butter is anhydrous, so it belongs to the same family, but it usually contains little or no wax, which means it has no set crystal network. It behaves as an aerated soft fat: light on the skin, quick to collapse in heat, and prone to grain if it was cooled slowly.

Which is best for very dry skin, a balm or a cream?

They do different jobs and work well together. A cream supplies water and humectants; a balm slows that water leaving. On badly dry skin the usual sequence is a humectant-containing cream or damp skin first, then a thin balm layer over the top to hold it. A balm alone in very low humidity is occlusion without anything to occlude.

Do salves and ointments need preservatives?

Not if they are genuinely water-free, because there is not enough available water for microbes to grow. They do need one as soon as water is present or routinely introduced: aloe juice, a hydrosol, honey, incompletely dried herbs, or shower-shelf use with wet fingers.
