Ingredients for chapped lips: what helps, what hurts, and why
Two evidence-graded lists: the occlusives and emollients that reliably help chapped lips, and the actives, flavours and filters that commonly make them worse.
Lips are a poor place to apply anything clever. The vermilion has a thin stratum corneum, effectively no sebaceous or sweat glands, and a rich nerve supply, so it loses water faster than surrounding skin, has no oil film of its own to replace, and reports every irritation immediately. That anatomy sets the whole ingredient argument: on lips, protection outperforms activity, and short lists outperform long ones.
The ingredients that reliably help chapped lips work by physics: petrolatum, lanolin, beeswax, shea butter, dimethicone and squalane reduce water loss and soften the surface. The ones that commonly cause trouble work by sensation or chemistry: menthol, camphor, phenol, salicylic acid, strong flavours, cinnamon and citrus oils, propyl gallate and some sunscreen filters.
What helping actually means here
An ingredient can help a chapped lip in exactly two physical ways. It can slow water leaving the surface, which is occlusion, or it can sit between the loosened surface cells and make them feel smooth and flexible, which is emolliency. A third category, humectants, attracts water and holds it, but needs water present to work with. Those three roles are separated properly in occlusive, emollient and humectant, and almost every disappointing lip product is a case of buying the wrong one of the three.
Nothing in a cosmetic balm accelerates repair as a drug would. What a good balm does is remove the obstacle, by holding water in long enough for the tissue to do its own work undisturbed. That is an unglamorous mechanism, and it is why the ingredient list of the most effective products is usually the most boring one on the shelf.

Two of these deserve a note. Petrolatum is used as the standard vehicle in patch testing precisely because it so rarely provokes a reaction, which is a strong practical statement about its reactivity. And shea butter is the one on the list that can degrade in the tube, because unlike the hydrocarbons it contains unsaturated fatty acids that oxidise. A shea and seed oil balm has a real shelf life that the hydrocarbons on the list do not.

The pattern is that everything in the second table is there for a reason other than protection: sensation, flavour, scent, shelf life or filtering. None of it is inherently unacceptable, and plenty of people use all of it happily. Why hyaluronic acid cannot work in an anhydrous balm
Hyaluronic acid is a large polysaccharide that holds many times its own weight in water. To do that it has to be hydrated, meaning dissolved in a water phase where the chains can uncoil and form a gel. A balm has no water phase at all, by definition: that is what anhydrous formulation means. Powdered sodium hyaluronate stirred into oil and wax does not dissolve. It sits there as suspended particles, and particles cannot form a hydrated film.
There are two partial defences and both are weak. The first is that it will hydrate on contact with moisture on the lip. There is not much moisture on a chapped lip, that is the whole complaint, and the powder is trapped under an occlusive film that is specifically designed to stop water reaching it. The second is that it is present at a low enough level to be harmless, which is true and is also an argument that it is doing nothing. The same logic applies to any water-loving humectant in a balm, glycerin included, though glycerin at least dissolves partially and can feel different in the mouth.
This is not an argument against hyaluronic acid. In a water-based lip serum applied under a balm, it has a real mechanism. It is an argument against reading it on the ingredient list of a wax stick and concluding the stick is more hydrating than the one without it.
Repair product versus comfort product
Most confusion about lip balms comes from expecting one product to do two different jobs. They are separable and it helps to name them.

The overnight case is the strongest argument for owning a repair product, because nobody is looking at you and the film stays undisturbed for seven hours. A thick layer of a plain occlusive before bed does more than a dozen daytime applications of something light. That approach, taken further, is what an overnight lip mask is.
SPF on lips, and the trade off nobody mentions
The lower lip receives a great deal of direct sun and has little natural protection, and cumulative damage there is a genuine dermatological concern rather than a cosmetic one. So a filtered balm outdoors is sensible. Two practical caveats follow. First, sun protection factors are measured with sunscreen applied at a defined density of 2 milligrams per square centimetre, which is a far heavier layer than most people apply from a stick, so real-world protection is lower than the number. Second, filters are a documented cause of lip reactions.
The workable compromise is two products: an unfiltered bland balm for indoors and overnight, and a filtered one used generously and reapplied for time outdoors. The exfoliation trap
Chapped lips flake, flakes look removable, and removing them feels productive. It is the single most common way people extend a problem that would otherwise resolve. The loosened layer is disorganised but it is still cover, and taking it off exposes tissue that was not ready, which flakes again a day later. Sugar scrubs, dry brushing, peeling with fingernails and acid-containing lip treatments all do the same thing at different speeds.
The alternative is duller and works: soften with a heavy occlusive, wait, and let the flakes lift and fall off on their own. If a flake is genuinely loose after a few hours under balm, it will come away against a soft cloth without resistance. Anything that needs pulling is not ready. When persistent chapping needs a clinician
No balm ingredient addresses a medical cause, and choosing between them stops being useful past a certain point. Seek assessment if chapping persists beyond two to three weeks on a bland regimen, if it is confined to the corners of the mouth, if there is bleeding, crusting, swelling or ulceration, if there is a persistent rough, white or scaly patch on the lower lip, if it began with a new medication, or if lip symptoms come with dryness of the eyes and mouth generally. Contact allergy is confirmed by patch testing, which a clinician arranges.
If you take one thing from the two tables, make it this: on lips, the ingredient that does the most is almost always the least interesting one in the formula, and the ingredients that generate the most enthusiasm on the front of the pack are disproportionately represented in the trouble list. Build the daily choice around a short list you tolerate, keep a heavy plain occlusive for the bad weeks, and when a product is not working, change one variable at a time. Frequently asked questions
What is the single most effective ingredient for chapped lips?
Petrolatum, on the physical evidence. It reduces water loss from the surface more than any other single material in routine use, it is chemically inert so it does not oxidise, and it has an unusually low rate of allergic reactions. It contributes nothing nutritive and it feels greasy, which are the honest trade-offs against it.
Does hyaluronic acid work in a lip balm?
Not in an anhydrous one. Hyaluronic acid is a humectant that has to be dissolved in water to form the hydrated gel that does the work. A balm contains no water phase, so the powder sits undissolved in oil and wax. In a water-based lip serum it can hold water at the surface, but under a balm it is close to label decoration.
Is menthol bad for chapped lips?
It does not help them. Menthol produces a cooling sensation by acting on a cold receptor, which feels like relief without changing the state of the tissue, and at higher levels it is a recognised irritant on thin skin. If lips are already inflamed, adding a cooling agent gives you sensation instead of protection and can prolong the problem.
Should a lip balm have SPF?
For outdoor use, yes. The lower lip is a common site for cumulative sun damage and it has very little natural protection. Indoors it matters much less, and sunscreen filters are a documented cause of lip reactions, so keeping a plain balm for indoor use and a filtered one for outdoors is a reasonable compromise.
Should I exfoliate chapped lips?
Generally no. The flaking layer is disorganised but it is still cover, and scrubbing it away exposes the tissue underneath and restarts the cycle. Softening with an occlusive and letting the flakes lift on their own is slower for about a day and better after three. Persistent scrubbing is a common reason chapping refuses to settle.
What is the difference between a repair balm and a comfort balm?
A repair product is built around a high load of occlusive with the shortest possible ingredient list, no flavour and no actives, and it is worn thickly and overnight. A comfort product is built for glide, gloss and taste, and is reapplied often through the day. Most people need both, and the mistake is expecting one product to do both jobs.
