Lip plumping balm: what a counter-irritant does, and where the claim line sits

A lip plumping balm with counter-irritant levels in percent, a sting threshold, patch testing, a tube pour band and an honest read on how long the effect lasts.

A plumping balm works by mildly irritating the lip on purpose. The tingle is real, the flush is real, and the mechanism behind both is well described; what is not real is added volume. This page gives the actives at working levels, the base that holds them in place, the patch test that should come before anyone wears it, and the exact wording that turns a cosmetic into a medicine.

Short answer

A plumping balm runs 0.3 to 0.8% vanillyl butyl ether, 0.2% menthol and about 0.1% ginger or capsicum extract in a base of 14% beeswax, 25% castor and 18% butters. Pour 4.5 g tubes at 66 to 72 C, with actives added below 50 C. The effect is transient vasodilation and mild oedema lasting roughly 10 to 30 minutes, not volume.

What actually happens to a lip

Three receptor families are involved and they do different things. Menthol opens TRPM8, the cold receptor, so the nerve reports cold that is not there. Vanillyl butyl ether and capsaicin act on TRPV1, the heat and pain receptor, producing warmth and, at higher doses, burning. Gingerols act on TRPV1 as well, more weakly and more slowly.

What produces the visible change is the axon reflex. Stimulating those sensory endings releases neuropeptides locally, small vessels in the dermis dilate, and blood flow to the vermilion rises. Two things follow. The lip reddens, and because red lips against surrounding skin read as fuller, part of the effect is straightforwardly optical. And a little fluid moves out of the dilated capillaries into the tissue, which is mild oedema: a genuine, tiny volume increase of the kind any local irritant produces.

That is the whole mechanism. Nothing has been added, no tissue has been built, and no collagen is involved. The change reverses as soon as blood flow returns to baseline. Independent controlled measurement of lip volume after a topical counter-irritant is thin, and most figures in circulation come from manufacturer-run panels using their own imaging, so treat any specific percentage increase as a marketing number. What can be stated with confidence is the mechanism, the direction and the duration, and the background is in menthol, camphor and other actives.

The actives, and the levels that work

Vanillyl butyl ether is the workhorse of the category: a clear, oil-soluble liquid related to vanillin that produces warmth without the pungency of capsaicin, and without capsaicin's habit of transferring to fingers and then to eyes. It is used across roughly 0.1 to 1 percent in leave-on cosmetics. On the lip, 0.3 percent is gentle, 0.8 percent is assertive, and above about 1 percent testers stop calling it a tingle.

Menthol at 0.2 percent is not there to plump. It is there because a cold signal arriving alongside a warm one is far more interesting than either alone. That figure sits at the top of the 0.05 to 0.3 percent band used for a cooling note in ordinary lip balm, and well below the 1.25 to 16 percent counterirritant range in the US external analgesic monograph, a distinction that matters and is set out in menthol and camphor safety limits.

Ginger or capsicum extract at 0.1 percent is the third note, added for slow warmth that builds after the menthol has faded. Standardised capsicum oleoresin is potent and unforgiving: a supplier may sell it at anything from 0.5 to 10 percent capsaicinoids, so 0.1 percent of one product is not 0.1 percent of another. Always work from the stated capsaicinoid content, not the extract weight. Ginger root extract is milder and easier to control, and it is the sensible choice for a first batch.

Careful

Capsicum materials are on regulators' watch lists and the entries change. Before you use one, check the current Annexes II and III of the EU Cosmetics Regulation for your European market, the Health Canada Cosmetic Ingredient Hotlist for Canada, and your own market's equivalent. A material that is acceptable in one jurisdiction may be restricted or prohibited in another, and that is a question for safety assessment and the CPSR, not for a recipe.

The formula

Weight percent of the total, adding to exactly 100. The base is deliberately firm and film-forming, because a plumper only works if the actives stay in contact with the vermilion rather than spreading and thinning out.

At 50 g the ginger extract is 0.05 g and the menthol 0.10 g, which most domestic scales cannot weigh to better than plus or minus 20 percent. On an active whose whole job is to sit just below a sting threshold, that is not a rounding error, it is the difference between a tingle and a complaint. Make the 250 g batch, or buy a scale reading to 0.001 g.

Why the base is built this way

14 percent beeswax alongside 18 percent butters gives a stick a little firmer than an ordinary tube balm, and the firmness is functional. A soft balm spreads past the vermilion border onto surrounding skin, which wastes active and puts a counter-irritant on facial skin that did not ask for it. A firmer stick lays a defined film where you press it.

25 percent castor oil is high for a balm and deliberate. Castor is 85 to 90 percent ricinoleic acid and it clings; the actives go where the film goes, so a film that stays put for an hour delivers a longer, gentler exposure than one that has been licked off in ten minutes. It also carries the gloss that does the optical half of the job.

18 percent butters, split between cocoa and shea, give the film substance without grain. Cocoa alone reads chalky and shea alone grains easily, a fault diagnosed in grainy shea butter. Method, and the temperature conflict nobody mentions

Here is the problem. Beeswax at 14 percent needs 66 to 72 C to pour a tube cleanly, and menthol sublimes readily while vanillyl butyl ether and the extract are both better off cool. The usual advice, add actives below 50 C, cannot be followed literally if you then have to reheat to 70 C to fill.

The resolution is that loss is driven by time and exposed surface area, not by crossing a threshold. Menthol does not vanish the instant a melt passes 50 C; it evaporates steadily from an open surface, faster the hotter it is. So you hold the actives in a small cool side portion, combine late, and keep the total time above 50 C to a few minutes with the jug covered.

  1. Weigh phase A, less a side portion. Set aside 10 percent of the batch weight of the caprylic/capric triglyceride and castor oil as a side portion. Everything else in phase A goes into the main jug.
  2. Melt at 75 to 80 C. Hold in a water bath at 75 to 80 C (167 to 176 F) until completely clear, with no shea crystals left. Cover the jug.
  3. Dissolve the actives cool. Warm the side portion to 45 to 50 C, off the heat, and stir in the menthol crystals until the liquid is completely clear. Then the vanillyl butyl ether, the ginger extract, the tocopherol and the flavour. Confirm it is clear before it goes anywhere near the wax.
  4. Combine and pour immediately. Bring the main melt down to 70 C, stir the active portion in for 30 seconds, and pour at 66 to 72 C (151 to 162 F). Aim to be filling within three minutes of combining.
  5. Stir between rows. Give the jug a slow turn with the spatula every four or five tubes. The actives are a small mass in a large one and a jug left to stand delivers a different dose to the last tube than the first.
  6. Top up, then cure. Wait 10 to 15 minutes for a shrinkage dip to form, pour a thin second layer, cool upright, then cure 24 to 48 hours before testing on anyone.

The pour band is the same as any 14 percent beeswax stick, and for the same reasons, which are set out in pour temperatures.

Try this

Weigh one tube from the start of the pour, one from the middle and one from the end, then test all three on the same person on different days. If the last tube stings noticeably more than the first, your stirring is the problem, not your percentage. Record the result against the batch code.

The sting threshold, and how to find yours

There is no published sting threshold in units, because the sensation depends on the individual, on how chapped the lip already is, and on how much was applied. What exists instead is a reproducible way to find the threshold for your formula, using a small panel and a fixed dose.

Run the ladder blind if you can, one level per day, with the same 0.05 g dose applied with a clean spatula rather than the stick, and ask testers to describe the sensation rather than rate it out of ten. Patch testing, in two steps

A counter-irritant is designed to provoke a response, so you cannot judge irritation by asking whether the lip feels anything. It will. The question is whether the response settles, and whether it is irritation or allergy.

  1. Forearm, 48 hours. Apply about 0.05 g to a marked 2 cm patch on the inner forearm, leave uncovered, and repeat once a day for two days. Expect mild transient redness for 10 to 20 minutes after each application. What you are looking for is anything still present at the 24 hour mark: persistent redness, itching, small papules or spreading beyond the marked area. Any of those means stop.
  2. A strip of lip, once. If the forearm is clear, apply a thin film to a 1 cm strip of the lower lip only, and wait 30 minutes. The lip is far more permeable than forearm skin and reacts at lower doses, so a clear forearm does not license a full application. Only after this passes should anyone wear the product normally.

Do not use an occlusive dressing for a counter-irritant patch test. Occlusion raises absorption substantially and will produce a reaction that tells you nothing about ordinary use. Where the claim line sits

This is the category that gets small makers into trouble, because the honest description of the effect sounds weaker than the product feels, and the tempting description is a drug claim.

In the United States a product becomes a drug when it is intended to affect the structure or any function of the body. In the EU and UK a cosmetic is defined by its purpose in Article 2 of Regulation (EC) No 1223/2009, and claims must additionally meet the common criteria of Regulation (EU) No 655/2013, which require a claim to be supported by adequate and verifiable evidence. Both systems judge intent by what you say, so the same stick is a cosmetic or an unapproved drug depending on the label and the caption under your photograph.

Two details catch people out. A comparative claim against an injectable is read as a claim about the injectable's effect, which is why "like filler" is worse than it sounds. And a before-and-after photograph is itself a claim: if the image shows more volume than the product delivers, the wording beside it will not save you. Who should not use it

A counter-irritant needs an intact surface to be merely uncomfortable rather than painful. On broken skin the same dose reaches deeper, faster.

  • Anyone with cracked, split or bleeding lips. The product will hurt and it will not help. Chapping wants an occlusive film, which is the argument in ingredients for chapped lips.
  • Anyone with an active cold sore, angular cheilitis or perioral dermatitis. Angular cheilitis in particular is aggravated by irritants, as cracked corners of the mouth explains.
  • Anyone who has recently had a lip procedure. The practitioner's aftercare instructions come first, always. Background in balm after lip filler.
  • Anyone with a known sensitivity to fragrance materials, balsams or vanillin relatives, and anyone who reacted to the patch test.
  • Children, at any age. Not a cautious house rule.
Careful

Children's lip products get no counter-irritants at all. Lip balm is eaten in small quantities as a matter of course, a child cannot report the difference between a tingle and pain in a way you can act on, and menthol, camphor and eucalyptus should not be applied near the face of children under two because of the risk of laryngospasm. The honest limits

Say the true thing and it still sells: it makes lips look fuller for about half an hour by flushing them, it feels like it is working because it is mildly irritating them, and it wears off. What it does not do is add volume, and any formula that appears to do more is doing more irritation, not more plumping. The only route from 0.6 percent vanillyl butyl ether to a different result is upward through the dose, and that is where the complaints and the sensitisation are.

Daily use is the part worth thinking hardest about. A counter-irritant applied several times a day to lip skin with no sebaceous protection and a thin stratum corneum is a low-grade provocation repeated indefinitely, and it encourages the reapplication loop described in does lip balm dry your lips. Position it as an occasional product, say so on the label, and put the two-step patch test in the instructions rather than the small print. If a tester's lips are worse after a week, the formula is not for sensitive skin and no soothing extract will make it so. Do lip plumpers actually work?

They produce a real, visible change and it is not extra volume. The result looks fuller for roughly 10 to 30 minutes and then returns to baseline. Nothing is added, built or stimulated.

How much vanillyl butyl ether goes in a lip plumper?

0.3 to 0.8 percent by weight, with 0.6 percent a good working level alongside 0.2 percent menthol. At 0.1 percent it is barely detectable. At 1 percent and above, testers stop describing the sensation as tingling and start describing it as burning, and a proportion of them wipe the product off, which is your signal to reformulate downwards.

Why does my plumping balm burn instead of tingling?

Usually one of three things. The total active load is too high, so drop the warming active. Menthol crystals were stirred into a thickening melt and never dissolved, leaving hot spots. Or a new lot of capsicum extract is stronger than the last, because suppliers sell anything from 0.5 to 10 percent capsaicinoids under the same name. Work from stated capsaicinoid content.

Can I say my balm plumps lips on the label?

Describing appearance is generally acceptable, for example that it gives lips a fuller look. Claiming it increases lip volume, stimulates collagen or replaces filler describes an effect on the structure or function of the body, and in the US that makes it a drug. How do I patch test a balm that is meant to sting?

In two steps, because a normal patch test cannot distinguish the intended sensation from a reaction. Apply about 0.05 g to a marked patch on the inner forearm daily for two days, uncovered, and look for anything still present at 24 hours. If that is clear, apply a thin film to a 1 cm strip of lower lip and wait 30 minutes before wearing it normally.

Is a plumping balm safe to use every day?

It is a mild irritant applied to skin with no sebaceous protection and a thin stratum corneum, so daily use is provocation repeated indefinitely. Some people tolerate it and some develop sore, flaky lips within a week. Treat it as an occasional product, stop at the first sign of persistent redness, and never use it on cracked or split lips.

Can children use a tingling lip balm?

No. Lip balm is eaten in small quantities as a matter of course, children cannot reliably report the difference between a tingle and pain, and menthol, camphor and eucalyptus should be kept away from the face of children under two because of the risk of laryngospasm.
