Foot and heel balm: a rich formula, and the honest limits of an anhydrous balm on cracked heels
A heavy foot balm formula by weight percent with grams for a 240 g batch, an optional menthol version, the sock and file routine, and where urea and salicylic acid beat a balm.
Heel balm is the product where makers most often overpromise. The skin on a heel is the thickest on the body, it has no sebaceous glands, and it is loaded in compression every time you stand up. That combination produces a specific failure, the fissure, and an anhydrous balm addresses part of it very well and another part hardly at all. This page gives a rich formula and is straight about the line between them.
A heel balm should run 8 to 12 percent wax with 40 percent or more hard fats and lanolin, applied at night under cotton socks. Occlusion softens skin and stops fissures reopening. It does not remove thickened callus: that needs filing and a keratolytic such as urea or salicylic acid.
Why heels crack, in mechanical terms
The plantar stratum corneum is many cell layers deeper than the stratum corneum on a forearm, and it thickens further in response to pressure. That is a useful adaptation until the tissue dries out. Dry keratin is stiff, and stiff material under tensile load does not stretch, it splits.
Every step spreads the heel fat pad sideways, putting the skin at the rim of the heel into tension. Where a thick, dry callus meets thinner skin, the strain concentrates at that boundary, which is exactly where fissures appear. Open-backed shoes make it worse because the fat pad has nothing to push against, so it spreads further with each step.
Two properties therefore matter: how much water the tissue holds (which sets its stiffness) and how thick the inelastic callus is. A balm moves the first. Only abrasion or a keratolytic moves the second, and that distinction runs through the whole page. The general version of the mechanism is in moisturisers and the skin barrier.
What a balm does well, and where it stops
An anhydrous balm contains no water. It works entirely by occlusion: a hydrophobic film that slows transepidermal water loss so the tissue underneath rehydrates from within. On plantar skin under a sock, that film stays put for eight hours, which is close to the ideal case for occlusion. Over a fortnight the skin becomes noticeably more pliable and fissures stop reopening, which is the outcome most people actually want.
What the balm cannot do is dissolve corneodesmosomes. Thickened callus sheds only when the bonds between corneocytes are broken, which is what urea above about ten percent and salicylic acid do chemically, and what a file or pumice does mechanically. No amount of shea butter substitutes for either.

The sensible routine uses more than one of these, in order: file gently, apply the balm, put socks on. If you sell a balm, note that the moment you claim it treats fissures or removes callus you have made a drug claim in the US, whatever the ingredients are. If you have diabetes, peripheral neuropathy or poor circulation, do not self-treat cracked heels. Reduced sensation means a fissure can deepen or become infected without pain to warn you, and foot ulcers are a serious complication. Have your feet assessed by a podiatrist or your diabetes team, and do not file callus yourself. Anyone with a fissure that is bleeding, weeping, hot, spreading or foul smelling needs a clinician now, not a balm.
The formula: heavy occlusive heel balm
Percentages are by weight and total exactly 100. The grams column is for a 240 g batch, which fills two 4 oz (120 mL) jars at 110 g each (220 g) with 20 g of overage, about nine percent, for the jug and spatula.
One figure in the table runs against the craft consensus. The mixed tocopherols go in at 0.1 percent rather than half a percent, because the measured optimum for alpha-tocopherol in an oil is about 100 parts per million and above that it starts to act as a pro-oxidant, so more is not better. Treat 0.05 to 0.2 percent of a mixed concentrate as the working range: the evidence is in vitamin E and antioxidants.

- Melt the hard phase. Beeswax, lanolin, shea and cocoa butter in a water bath. Hold the batch at 75-80 C (167-176 F) for ten minutes. Cocoa butter is polymorphic and must be fully melted or it sets with pale streaks.
- Add the liquid oils. Castor, high-oleic sunflower and meadowfoam. Return to 75 C and stir until uniform. Lanolin disperses slowly, so keep stirring until there is no separate glossy layer.
- Cool to 58-60 C (136-140 F). Off the heat, stirring slowly to avoid folding in air.
- Add tocopherol and fragrance at 58-60 C. Stir briefly, no more than ten seconds.
- Pour at 60-65 C (140-149 F) to 110 g per 4 oz jar.
- Cool fast. Twenty minutes in the fridge, not the freezer. A shea and cocoa butter blend cooled slowly on a worktop is the classic route to grain.
- Cure 24 hours, then cap. The balm reads noticeably firmer on day two than on day one.
How to adjust it
One change per batch, always rebalanced against the liquid oils so the total stays at 100.
- Firmer, for a warm climate: beeswax 10 to 12, take the 2 points from high-oleic sunflower.
- Softer, easier to work into a rough heel: beeswax 10 to 8 and cocoa butter 12 to 9, adding the 5 points to sunflower.
- More occlusion: lanolin 15 to 20, taking the 5 points from sunflower. Above about 20 percent the balm gets genuinely sticky, which is fine under socks and unpleasant otherwise.
- Lanolin free: replace the 15 percent lanolin with 8 percent castor (raising castor to 23), 4 percent cocoa butter and 3 percent meadowfoam. The seal is weaker; nothing plant based matches lanolin gram for gram.
- Tallow version: swap shea and cocoa butter for 27 percent rendered tallow, keeping everything else the same. Tallow melts at 40-49 C and gives a heavier, more clinging film: see tallow.
- Fragrance free: drop the essential oil and add 1.5 points to sunflower. Recommended for anyone with fissures, since open skin is where sensitisation happens.
The optional cooling version
A menthol foot balm is a comfort product, not a moisturising upgrade. Menthol activates cold-sensing receptors, so the foot feels cool without changing temperature. Add 1 percent menthol crystals (2 percent if you want it emphatic), dissolved into the warm oil phase at 50-55 C before pouring, and take the points from high-oleic sunflower. Camphor at 1 percent is sometimes added alongside it.
Menthol and camphor should not be applied to or near the face of children under 2, and strong menthol or camphor products are not for infants. Do not use a mentholated balm on open fissures, broken skin or immediately after filing: it stings sharply. In the US, menthol and camphor above the levels used for scent are regulated as active drug ingredients rather than cosmetic ones. The sock and file routine
The balm is one part of a sequence, and the sequence outperforms any formula used on its own.
- Soak for 10 minutes. Warm water, no soap needed. Hydrated callus files evenly; dry callus tears.
- File lightly, on damp skin. A foot file or pumice, moderate pressure, one direction, 30 seconds per heel. Stop while there is still callus left. Removing too much in one session leaves tender skin that cracks again within days.
- Pat dry, leave slightly damp. Occlusion works best over skin that has water in it to trap.
- Apply about 1 g per foot. Work it into the rim of the heel and the ball, not just the flat centre.
- Cotton socks, immediately. The sock is doing half the work: it stops the balm transferring to the sheets and keeps the film intact for eight hours.
- Repeat nightly for two to three weeks, then drop to three or four nights a week. File once a week at most.

- 4 oz (120 mL) jar: 105-112 g fill. 2 oz (60 mL) tin: 52-56 g.
- Working density for volume to weight: 0.92 g/mL.
- Typical nightly use: 1-1.5 g per foot, so a 110 g jar is about 40 nights for two feet.
- Batch overage to allow: 8-15 percent.
Why this balm is heavier than a body balm
Compare the numbers. A body balm sits at 5-12 percent wax with 20-40 percent butters, tuned to disappear within a minute on skin that has sebaceous glands of its own. This formula runs 42 percent hard fats plus lanolin, and it is designed to still be detectable eight hours later. That is not a richer version of the same thing, it is a different target: plantar skin has no sebaceous glands, so there is no native lipid film for the product to supplement. Where to start
Make the 240 g batch unscented, use it nightly under socks for three weeks, and judge it on one question: are the fissures still reopening? If they are not, the balm is doing its job and the remaining callus is a filing problem. If the heel is still splitting despite consistent nightly use, the answer is not a richer balm, it is a keratolytic in the routine or a podiatrist. Knowing which of those two the product is for is most of what separates a good foot balm from an optimistic one. Frequently asked questions
Will a balm heal cracked heels?
A balm softens and seals, which makes fissures more comfortable and much less likely to reopen, and it is the best thing to use nightly under socks. It does not remove thickened callus. If the hard rim around a fissure is the problem, a keratolytic product containing urea or salicylic acid, plus mechanical filing, is what actually reduces it.
What is the difference between a foot balm and a urea cream?
A foot balm is anhydrous: fats and wax that seal water in. A urea cream is water based and urea is both a humectant and, above roughly ten percent, a keratolytic that loosens the bonds between corneocytes so thickened skin sheds. They do different jobs and work well in sequence, urea first, balm on top overnight.
How much wax should a foot balm have?
Eight to twelve percent by weight. Foot skin is thick and insensitive to drag, so you can carry far more hard fat here than on a face balm, but wax above about twelve percent stops the balm melting fast enough under a thumb and it ends up smeared rather than worked in.
Is peppermint or menthol necessary in a foot balm?
No. Menthol produces a cooling sensation by acting on cold receptors, which many people like after a day standing, but it does nothing for dryness and it stings badly on an open fissure. Keep it to one to two percent, leave it out of the version you use on cracked skin, and keep it away from young children entirely.
Why do heels crack in the first place?
The plantar stratum corneum is many times thicker than skin elsewhere and has no sebaceous glands. Pressure spreads the heel pad sideways with every step, and thick, dry, inelastic skin splits under that load rather than stretching. Open-backed shoes, standing on hard floors and low humidity all make the split more likely.
Can I use the same balm on hands and feet?
You can, but the design points differ. A foot balm can be tackier, heavier and slower to absorb than anything you would tolerate on a hand, because it goes on under a sock and stays there. Used on hands it will feel greasy for far too long.
How often should I apply it?
Nightly for the first two to three weeks, then three or four nights a week to hold the result. Skin comfort improves within days, but the visible state of a thickened heel reflects weeks of turnover, so judge the routine at a month rather than at a weekend.
