Lanolin: chemistry, occlusivity, allergy and how to use it

Anhydrous lanolin explained: sterol esters not triglycerides, why it beats most fats for chapping, the allergy question, grades, tack and usage rates.

Lanolin is the waxy secretion sheep produce to waterproof their own fleece, recovered from the water used to scour raw wool. It is the oldest emollient still in mainstream medical use, and it is chemically unlike every other fat in a balm maker's cupboard. That difference is why it behaves the way it does.

Short answer

Anhydrous lanolin is a wax, not an oil. It is built from sterol and alcohol esters rather than triglycerides, melts at 38 to 44 C, and is among the most occlusive natural materials available. It takes up twice its weight in water. Contact allergy runs at about 0.4 percent in the general population and far higher on chronically broken skin.

What lanolin actually is

Almost every other fat in a balm is a triglyceride: three fatty acids on a glycerol backbone. Lanolin has essentially no glycerol and no triglycerides. It is roughly 95 percent esters formed between long-chain lanolin acids and a set of alcohols that includes cholesterol, lanosterol and other sterols, plus aliphatic and triterpenoid alcohols. The acid fraction is unusual too: much of it is branched (iso and anteiso) or alpha-hydroxy substituted, which is why lanolin stays soft and sticky over a wide temperature range instead of setting into a crisp crystal like beeswax.

The remaining few percent are free lanolin alcohols, sometimes called wool alcohols. That small fraction carries most of the material's water-holding capacity, and most of its allergy history.

Cholesterol is worth pausing on. It is one of the three lipid classes that build the stratum corneum barrier, alongside ceramides and free fatty acids, so lanolin is one of the few common balm ingredients that shares real structural chemistry with skin. That is a stronger version of the claim often made for animal fats generally, and it is covered from the skin side in moisturisers and the skin barrier.

Why it works on badly chapped skin

Lanolin does three things at once, which is unusual.

  1. It occludes. The film is continuous, flexible and slow to rub off, so transepidermal water loss falls sharply and stays down. On lips, which have no sebaceous glands to speak of and lose water fast, this is the whole mechanism.
  2. It holds water. Anhydrous lanolin will take up around twice its own weight of water and hold it as a water-in-oil dispersion. In practice it can trap moisture already at the skin surface rather than only sealing it in, which is why it feels different from a purely inert occlusive.
  3. It stays. Its tack is a functional property. A lanolin film survives lip movement, hand washing and bedding contact longer than a wax and oil balm of similar hardness.

How it compares with the other heavy hitters

Relative performance of the main occlusive balm materials. Occlusivity is a qualitative ranking from the published emollient literature, not a measured site figure. Cost bands are relative to each other by weight, not shop prices.

Read that table as a division of labour rather than a league table. Beeswax is in a balm to hold the shape; shea butter is there for cushion and skin feel; lanolin and petrolatum are there to stop water leaving. A well-built repair balm usually contains something from at least two of those columns.

Grades: USP, medical and ultra-refined

Lanolin is sold at several purities, and the labels are not standardised across suppliers. What follows is the practical hierarchy.

Lanolin grades in ascending order of purification. Free lanolin alcohol content is the property most relevant to sensitisation.

There are also modified lanolins. Lanolin oil is the liquid fraction separated from the solid, and it is thinner and much less tacky. Acetylated lanolin is chemically modified to remove the hydroxyl groups, which kills both the tack and the water-holding. Lanolin alcohol is the alcohol fraction on its own, used as an emulsifier and, unhelpfully for anyone worried about allergy, the fraction that carries most of the sensitising potential. The allergy question, with the real numbers

Lanolin's reputation as an allergen is partly earned and partly inherited from a version of the material that is no longer sold. Mid twentieth century lanolin carried detergent residues from wool scouring, pesticide residues from sheep dipping, and a higher proportion of free lanolin alcohols. It became a standard entry on patch test series, where it remains, which is fortunate: it means there are decades of population data rather than opinion.

Reported rates of positive patch tests to lanolin or wool alcohols, by population. Clinic figures are drawn from people referred for suspected contact allergy and are enriched for eczema and chronic wounds, so they are not general population rates. Set those against the allergens sitting beside lanolin on the same test series: nickel at 16 to 25 percent, methylisothiazolinone at 11 to 15 percent, fragrance mix I at roughly 9 to 13 percent. Lanolin has never been a top-tier allergen. It is on the standard series because it is common in emollients applied to damaged skin, not because it sensitises a large share of the people who meet it.

Three findings that explain the reputation

  • The lanolin paradox. Wolf, Dermatology 1996;192:198 (PMID 8726630), set out the pattern: lanolin in medicated products sensitises a notable share of patients, while the same material is unremarkable in cosmetics used by far more people. The same work concluded that patch testing with 30 percent wool wax alcohol "cannot be considered a reliable method", carrying too many false positives and false negatives.
  • The allergens are free fatty alcohols and detergent residues, not lanolin as such. Clark, Cronin and Wilkinson, Contact Dermatitis 1977;3:69 (PMID 872577), found that removing both cut detectable hypersensitivity in sensitive patients by 96 percent. A follow-up (PMID 7238016) brought free alcohols below 3 percent and recorded one reaction in 149 subjects, a 99.3 percent reduction. Modern highly purified grades keep free alcohols below 1.5 percent, which is why the grade in the jar matters more than the word on the label.
  • A use test is much more reassuring than a patch test. Uldahl, Engfeldt and Svedman, Contact Dermatitis 2021;84:41 (PMID 32844454), gave twelve patch-test positive subjects lanolin cream to apply twice daily for four weeks, to intact skin and to deliberately irritated skin. None of them reacted on intact skin, and all of them healed during the test on damaged skin.

Do not overshoot in the other direction, though. Between 83 and 87 percent of positive lanolin reactions in the North American data were judged to be of current clinical relevance, so these are not mostly laboratory artefacts, and a 2024 review (Lis, Life 2024;14:1553, PMID 39768261) notes that the evidence base is weak in both directions. Nobody has run the large trial that would settle it.

Where that leaves the advice

"Lanolin is a common allergen, avoid it" is overstated for intact skin. It is not overstated for broken skin, leg ulcers, stasis dermatitis, established eczema, or perianal and genital skin, where the reported rate rises several-fold and the cost of being wrong is higher. For anyone with a confirmed lanolin allergy, petrolatum is the evidence-backed alternative: the same occlusive job, done by the least reactive material in common use.

Safety

If you have long-standing eczema, a history of reacting to ointments, or open cracks you are treating daily, patch test a lanolin product on the inner forearm for several days before using it widely. If you are selling a lanolin balm, declare it clearly. It is a recognised allergen and some buyers avoid it deliberately.

Tack, and how to cut it

Lanolin's stickiness comes from the same structural features that make it work, so you cannot remove it entirely without removing the benefit. You can manage it.

  • Lower the percentage. Most of the cling appears in the first 5 percent. Doubling from 5 to 10 percent does not double the benefit but does roughly double the perceived stickiness.
  • Pair it with a dry-feel oil. Fractionated coconut, squalane or jojoba in the liquid phase pulls the finish back toward dry without diluting the occlusive film much.
  • Add hard structure. A little more wax or a firm butter turns a sticky paste into a firm balm that deposits a thinner layer.
  • Substitute part of it. Lanolin oil or acetylated lanolin at half the total lanolin load keeps some character and loses much of the drag.
  • Use the tack deliberately. In an overnight lip mask or a cuticle balm, stickiness is a feature: it means the product is still there in the morning.

Usage rates, from a trace to a whole product

Key numbers
  • 3-5 percent: adds gloss and cling to a lip balm, barely detectable as tack.
  • 8-15 percent: noticeable repair character in hand, heel and cuticle balms.
  • 20-30 percent: a deliberately sticky, heavy overnight or barrier product.
  • 100 percent: sold as-is for nipple and severe chapping use, usually an ultra-refined grade.
  • Melting range 38-44 C (100-111 F), so lanolin contributes body but very little hardness.

A worked example, in weight percent of the total, adding to 100. For a 50 g pot of hand repair balm: 10 percent lanolin (5.0 g), 12 percent beeswax (6.0 g), 25 percent shea butter (12.5 g), 53 percent high-oleic sunflower oil (26.5 g). Melt in a water bath at 70 to 80 C, add lanolin with the other solids because it needs full melting to disperse evenly, then pour at 60 to 68 C. Note that lanolin does not harden a balm: if you swap wax for lanolin one for one, the result will be softer.

Workshop tip

Weigh lanolin warm. Cold anhydrous lanolin is a stiff, stringy paste that clings to the spatula and will cost you a gram or two per batch. Stand the jar in warm water for ten minutes first, or account for it in your batch overage.

Sourcing, welfare and the vegan question

Lanolin is a by-product. Sheep are shorn for wool, the grease is washed out of the fleece during scouring, and it is recovered from the effluent. No animal is killed for it, and the alternative to recovering it is discharging it. On that basis it is vegetarian by most definitions and not vegan, and vegan formulators need a different route entirely: see vegan beeswax substitutes for structurants, though nothing plant-derived reproduces lanolin's specific combination of occlusion and water uptake.

The welfare issue that attaches to lanolin is mulesing, the removal of skin folds from the breech of Merino sheep to reduce flystrike, practised mainly in Australia and performed historically without pain relief. Alternatives exist, including breeding for plainer-bodied sheep, and non-mulesed certification schemes are available. If this matters to you or your customers, ask your supplier for the wool origin and the certification, and be aware that lanolin is often pooled across sources, so a confident answer is not always available.

When to reach for it

Use lanolin when the problem is water loss through damaged skin and nothing lighter has worked: split lips in February, cracked fingertips, heels, cuticles, nipples. Do not use it as a general-purpose emollient in a light daily balm, where its tack is a cost with no matching benefit, and do not use it at all in a product aimed at people who have told you they react to it. If you have never worked with it, buy a small amount of an ultra-refined grade, make a 5 percent lip balm and a 20 percent hand balm from the same base, and use both for a fortnight. The difference between those two will tell you more than any table can.

Frequently asked questions

Is lanolin good for lips?

It is one of the best single materials for badly chapped lips. It forms a strong occlusive film, it holds water rather than simply blocking it, and it stays put through talking and eating better than most oils. The downsides are a sticky feel, a faint sheep-like smell in some grades, and a small chance of contact allergy.

Is lanolin better than petroleum jelly?

They do slightly different jobs. Petrolatum is the stronger pure occlusive and is essentially inert. Lanolin is close behind on occlusivity but also takes up water, softens the skin surface more actively, and comes from a renewable source. Petrolatum has a lower allergy risk. Lanolin has the better feel and the better sourcing story.

Is lanolin safe?

For the great majority of people, yes. Pharmacopoeial and ultra-refined grades have a long record of use, including on infants and on nipples during breastfeeding. The main risk is contact allergy, which is uncommon in the general population and more frequent in people with long-standing eczema, leg ulcers or chronically broken skin.

How common is lanolin allergy?

About 0.4 percent in a random general population sample. In North American patch test clinics, which are enriched for eczema and chronic wounds, 3.3 percent tested positive across 2001 to 2018. It rises steeply on damaged skin: 6.0 percent in lower-leg dermatitis and 24.7 percent in one series of stasis dermatitis and leg ulcer patients. For scale, nickel runs at 16 to 25 percent in the same clinics.

Is lanolin vegan?

No. It is recovered from the wool of living sheep during scouring, so it is vegetarian by most definitions but not vegan. No animal is killed for it, and it would otherwise be a waste stream of the wool industry. The welfare question that matters is mulesing, which is about the sheep rather than about the lanolin.

How much lanolin should I use in a balm?

Start at 3 to 5 percent for cling and gloss without noticeable tack. Use 10 to 20 percent for a repair balm on hands, heels or cuticles. Above 25 percent the stickiness becomes the defining characteristic, which is exactly what you want in a nipple or overnight lip product, and wrong in a daytime one.

Why is my lanolin balm so sticky?

Because lanolin is intrinsically tacky: its sterol esters and free alcohols give a high-drag film. Cut it by lowering the percentage, blending with a dry-feel oil such as fractionated coconut or jojoba, adding a hard butter, or switching part of it to lanolin oil or acetylated lanolin, both of which are much less tacky.
