Nappy rash balm: the barrier principle, a zinc oxide variant, and the rashes a balm makes worse
A plain barrier balm and a 20 percent zinc oxide variant in weight percent and grams, how to spot a yeast-involved rash, and why heavy balms ruin cloth nappies.
Nappy rash is the clearest example on this site of a product whose job is entirely physical. The balm is not doing chemistry on the skin. It is a raincoat. Understanding that changes what you put in it, because almost every ingredient people are tempted to add is trying to do something else, and the nappy area of an infant is the worst place on a human body to experiment.
A nappy balm's only job is to keep urine and faeces off the skin. A plain occlusive of petrolatum, butters and wax does that. Zinc oxide at 10 to 40 percent adds an opaque protective layer, and in the United States makes the product an over-the-counter drug rather than a cosmetic. No essential oils, ever.
See a clinician for any rash that blisters, weeps, bleeds, crusts or ulcerates, spreads beyond the nappy area, has not improved after three to five days of good barrier care, is deep red and creeping into the skin creases, or comes with fever, unusual sleepiness or poor feeding. Also see one for a rash in a newborn under one month. A balm is a comfort and protection product. It does not treat infection, thrush, eczema or any other diagnosis, and describing it as doing so is a medicinal claim: see cosmetic versus drug claims.
The barrier principle
Irritant nappy dermatitis has a well described chain. A nappy holds urine and faeces against the skin. Urease-producing bacteria in faeces split urea and raise the local pH. At the higher pH, faecal proteases and lipases become considerably more active and attack the stratum corneum directly. Meanwhile the skin is over-hydrated, and over-hydrated skin has much lower resistance to friction. Wetness, pH and enzymes together do the damage; friction finishes it.
A barrier balm interrupts exactly one link in that chain, and it is the first one. A hydrophobic film sits between the skin and the wet mixture. Nothing about the balm is anti-inflammatory, antimicrobial or healing. It removes the insult and lets skin do what skin does. It is worth noting how directly this maps onto the regulatory picture. The US over-the-counter skin protectant monograph lists white petrolatum at 30 to 100 percent, mineral oil at 30 to 100 percent, cocoa butter at 50 to 100 percent, hard fat at 50 to 100 percent and lanolin at 12.5 to 50 percent as recognised active ingredients. A plain barrier balm is not a folk remedy standing in for a real product. It is made of the same materials the monograph names.
Formula A: plain barrier balm
Percentages are by weight and total exactly 100. The grams column is for a 240 g batch, filling two 4 oz (120 mL) jars at 110 g each (220 g) with 20 g of overage, about 9 percent.

There is no lanolin in it, deliberately. Lanolin is an excellent occlusive and a recognised skin protectant, but contact allergy rates rise where skin is broken, and nappy rash is a broken-skin situation on a person who cannot tell you it itches. Keep it for the intact-skin products.
Formula B: the zinc oxide variant
Zinc oxide is opaque, mildly astringent and adds physical bulk to the film. Many people find it works better on a sore, weepy area than a clear balm. Percentages are by weight and total exactly 100, again for a 240 g batch in two 4 oz jars.

Do not inhale zinc oxide dust. Weigh it slowly in a still room with no fan, wear a fitted dust mask, and never tip powder into an open pan from a height. Wet it into a small amount of oil first, so it never becomes airborne. Use a non-nano cosmetic or pharmaceutical grade: EU rules restrict nano zinc oxide from any application that could lead to inhalation exposure, which tells you where the concern lies. Keep the powder away from children and clean the bench with a damp cloth rather than a brush.
Method, including dispersing the zinc
- Sanitise everything. Wash, dry, then wipe with 70 percent alcohol and let it evaporate. Water is the enemy in an anhydrous product.
- Make the zinc slurry first (formula B only). Sieve the zinc oxide, then stir it into about a third of the fractionated coconut oil with a small spatula until it is a smooth, lump-free paste. Wetting the powder before it goes near heat is the whole technique.
- Melt the hard phase. Petrolatum, beeswax and shea butter in a water bath at 75-80 C (167-176 F) for ten minutes, so every shea crystal is destroyed.
- Add the remaining liquid oils, return to 75 C and stir until uniform.
- Stir in the zinc slurry and keep stirring. Zinc oxide is about six times denser than the oil around it, so it settles fast in a thin melt.
- Cool to 60 C (140 F) with continuous slow stirring, and add the tocopherol. Stirring through the setting range is what keeps the zinc evenly suspended.
- Pour at 60-65 C (140-149 F) to 110 g per 4 oz jar. Pour temperature logic is in pour temperatures.
- Chill 20 minutes in the fridge, not the freezer, then cure 24 hours before judging the texture.
In the United States, a product containing zinc oxide as a skin protectant is an over-the-counter drug, not a cosmetic. The monograph band for zinc oxide is 1 to 25 percent, and commercial "maximum strength" creams at 40 percent sit above it. Making formula B for your own family is fine. Selling it means a drug product with the labelling, facility registration and record obligations that follow, which is a different undertaking from selling a balm.

Telling an irritant rash from one that needs a clinician
This is the judgement that matters most, because a heavy occlusive is the right answer for the first pattern and an unhelpful one for the rest.

The reason the occlusive is wrong for the second and third patterns is straightforward: warmth and moisture under an impermeable layer are the conditions Candida likes, and a thick film also makes it harder to see what the skin is doing and harder for a prescribed cream to reach it. If a barrier product is not improving things within three to five days, the answer is a different diagnosis, not a thicker balm.
The things that fix more rashes than any product
- Change more often. Newborns need changing very frequently; older babies roughly every three to four hours and immediately after a dirty nappy. This lever outperforms every formula on this page.
- Nappy-free time. Ten to fifteen minutes on a towel a few times a day. Dry air on the skin does something no balm can.
- Clean gently. Water and cotton wool, or fragrance-free and alcohol-free wipes. Pat dry, do not rub, and let the skin dry fully before the balm goes on.
- Apply a thin film, not a plaster. A visible smear is enough. Thick layers trap heat, waste product and make the next change harder.
- Do not scrub the balm off. Leave what is still there in place at the next change and top it up.
Cloth nappies, and why heavy balms ruin them
Petrolatum, zinc oxide, lanolin and heavy butters all coat textile fibres. Once coated, the fibre repels water instead of wicking it, so the nappy leaks and the absorbency loss compounds over many washes. This is not a myth and it is not fixed by a hot wash. Use a disposable paper liner or a fleece liner between the balm and the nappy, or use disposables for the few days it takes a rash to settle. If you must use a cloth-compatible product, that means a much lighter, non-petrolatum, non-zinc formula, which is also a much weaker barrier. Be clear with yourself about which trade you are making.
- 4 oz (120 mL) jar: 105-112 g fill. 2 oz (60 mL) tin: 52-56 g.
- Zinc oxide in commercial nappy creams: 10 to 40 percent. US monograph band: 1 to 25 percent.
- Working density of a finished balm: 0.92 g/mL. Zinc oxide is far denser and will settle.
- Reassess if there is no improvement in three to five days.

What to make, and when to stop making things
Make formula A first, unscented, in a 240 g batch, and use it at every change as a preventive film rather than waiting for redness. Most nappy rash never gets past that. Add the zinc variant only if you want an opaque paste for a sore patch, and only once you have read the note about what it does to the product's legal status. Above all, keep the timeline in view: three to five days of consistent barrier care and more frequent changes should make a visible difference. If it does not, the next step is a clinician, not a better recipe.
Frequently asked questions
What does a nappy balm actually do?
It keeps moisture off the skin. Nappy rash is driven by prolonged contact with urine and faeces: the pH rises, faecal enzymes become more active, and wet skin is far more easily damaged by friction. A barrier balm puts a water-repellent film between the skin and that mixture. It is a physical shield, not a treatment for anything.
Is zinc oxide necessary?
No. A plain occlusive balm without zinc is enough for most mild irritant rashes, and it is easier to make, easier to wash off and easier to see the skin through. Zinc oxide adds an opaque, mildly astringent layer that many people find works better on a sore, weepy area. It also changes the product's regulatory status in the United States.
How much zinc oxide should I use?
Commercial nappy creams run from about 10 to 40 percent. Twenty percent is a sensible home target: clearly opaque and protective without being so stiff it drags on inflamed skin. Note that the US skin protectant monograph lists zinc oxide at 1 to 25 percent, so a 40 percent product sits above the monograph range.
How do I know if a rash is thrush?
Candida involvement usually looks different: a deep beefy red, sharply edged, going into the skin creases rather than sparing them, often with small separate spots scattered beyond the main edge. It commonly follows a rash that has lasted more than about three days or has not responded to barrier care. That needs a clinician and an antifungal, and piling a heavy occlusive over it is unhelpful.
Can I add lavender or tea tree oil?
No. Essential oils have no place in a product for the nappy area of an infant. The skin there is occluded, warm, often broken and highly absorbent, which is the worst possible site for a sensitiser, and the ingredients contribute nothing a barrier balm needs. A fragrance-free formula is not a compromise here, it is the correct design.
Will a homemade balm ruin cloth nappies?
Yes, if it gets into the absorbent layers. Petrolatum, zinc oxide, lanolin and heavy butters coat the fibres and make them repel liquid rather than absorb it, and the damage builds up over many washes. Use a disposable or fleece liner between the balm and the nappy, or switch to disposables while the rash is being treated.
