Muscle rub balm: a cooling formula, a warming formula, and what a counter-irritant really does
Two muscle rub formulas in weight percent and grams, maximum levels for menthol, camphor and capsicum, arnica in context, and how to dose a potent active evenly.
A muscle rub is a balm with a job that is easy to overstate. The base is ordinary: wax, a butter and oils, the same architecture as any other salve. What makes it difficult is that the interesting ingredients are potent at fractions of a percent, distribute badly in a setting wax matrix, and sit on the line between a cosmetic and a licensed medicine.
A muscle rub works by counter-irritation: menthol on cold receptors, capsaicin and ginger on heat receptors. It changes what you feel, not what the tissue is doing. A cooling rub at 8 percent menthol and a warming rub at 0.05 percent capsicum oleoresin are the two useful formats. Neither is anti-inflammatory.
What a counter-irritant does and does not do
Menthol binds TRPM8, the receptor that signals cold. Capsaicin binds TRPV1, which signals noxious heat. Both produce a strong sensation from a stimulus that is not changing skin temperature, and that sensation competes for attention with an aching muscle. Repeated capsaicin exposure also defunctionalises the nerve endings, which is why capsaicin products are dosed as a course.
What none of this does is reach a muscle. A balm penetrates the stratum corneum and upper epidermis. It does not deliver anything centimetres down, reduce swelling or accelerate repair. Calling a homemade rub anti-inflammatory, or a treatment for strains or arthritis, is wrong and is a medicinal claim: see cosmetic versus drug claims.
Pain after a fall or impact, pain with visible deformity, numbness, pins and needles, weakness, a joint that will not bear weight, calf pain with swelling or redness, chest pain, pain that wakes you at night, or pain worsening over weeks all need a clinician, not a balm. A counter-irritant that masks a warning signal is a hazard.
The counter-irritant family, with levels
These are the materials that turn up in commercial rubs, with the counterirritant bands from the US external analgesic monograph. That monograph is OTC Monograph M017, not 21 CFR Part 348, which is the citation you will see everywhere and which is wrong: Part 348 as codified covers only male genital desensitisers. A monograph band is not permission to use a material either. It tells you where a product stops being a cosmetic.

Notes on the cooling actives are in menthol, camphor and actives. Clove and cinnamon leaf are not a quiet way to add warmth either: they are largely eugenol, which IFRA restricts to 0.45 percent in lip products and 0.64 percent in body products, so they run out of headroom long before they warm anything. Methyl salicylate needs a section of its own.
Methyl salicylate: legal in a US rub, effectively banned in a European one
This is the most useful thing this page can tell a maker, and almost nobody publishes it. In the United States methyl salicylate is a counterirritant active at 10 to 60 percent under M017, which is where deep-heat rubs live. In the EU it is Annex III entry 324 of Regulation (EC) No 1223/2009, as amended by Regulation (EU) 2022/1531: 0.06 percent in leave-on skin products, 0.03 percent in lip products, and not to be used for children under 6.

Wintergreen and sweet birch oils are 85 to 99 percent methyl salicylate, so they cap out at roughly 0.06 to 0.07 percent of a leave-on balm. A US-style 10 to 60 percent rub is not a legal European cosmetic, and at those levels it is a US drug rather than a cosmetic anyway. No formulation trick bridges that gap, and calling the oil natural does not change the molecule.
Oil of wintergreen is a household poisoning risk, and a jar of muscle rub should be stored like one. The American Association of Poison Control Centers consensus guideline (PMID 17364628) says that more than a lick or taste of oil of wintergreen (98 percent methyl salicylate) by a child under 6, or more than 4 mL by anyone 6 or older, warrants emergency department referral. A teaspoon is about 5 mL, so one teaspoon carries a very large salicylate dose. Keep any salicylate-containing rub capped and out of reach, and call a poisons centre immediately after a suspected ingestion rather than waiting for symptoms.
Arnica, in context
Arnica montana has a long traditional use in bruising and soreness, and an arnica-infused oil is a historically grounded base. The evidence is more modest than the marketing: systematic reviews find few trials, of variable quality, with arnica gel among the better studied but not established. Arnica is a Compositae plant and a recognised contact sensitiser.
Arnica is for unbroken skin only. Not on cuts, grazes, open blisters, eczematous skin or mucous membranes, and never internally: arnica preparations are toxic if swallowed. Stop if the skin becomes itchy, bumpy or red, which is how Compositae sensitisation presents. Avoid it with a known daisy, ragweed or chamomile allergy.
Infuse fully dried flowers, never fresh. The hazard in an oil infusion is the water in plant tissue, not the oil. Method and ratios are in herbal infused oils, and the base architecture in herbal salve.
Formula A: cooling muscle rub
Percentages are by weight and total exactly 100. The grams column is for a 240 g batch filling four 2 oz (60 mL) tins at 55 g each (220 g) with 20 g of overage, about 9 percent.

Percentages are by weight and total exactly 100, again for a 240 g batch. Note the capsicum figure: 0.05 percent is 0.12 g here, which no domestic scale can weigh directly. That problem, not the recipe, is the real content of this section.

Capsaicin is the ingredient on this page most likely to hurt someone. Wear nitrile gloves throughout, keep it away from your face, and wash with soap and cold water afterwards, then again. It transfers readily from fingers to eyes, nostrils, lips and genitals, where it is agonising and lasts for hours. Never combine a warming rub with a heat pack, hot bath, sauna or occlusive wrap, and never apply it to broken skin. Keep it away from children and pets. If it gets in an eye, irrigate with plenty of water and seek medical advice.
Method, for either formula
- Melt the structuring phase. Beeswax and cocoa butter in a water bath at 75-80 C (167-176 F) for ten minutes, so no cocoa butter crystal survives.
- Add the oils. Infused oil, fractionated coconut and castor, back to 70 C (158 F) and stirred uniform.
- Cool to 50-55 C (122-131 F). Menthol and camphor sublime above roughly 60 C, sending active into the room instead of the tin.
- Dissolve the crystals (formula A), stirring to a completely clear melt, or stir in the capsicum predilution (formula B) at about 55 C, gloved.
- Add the essential oil and tocopherol at 50-55 C, stirring for ten seconds.
- Pour at 55-60 C (131-140 F) to 55 g per 2 oz tin, filling every tin in one continuous pass.
- Lid as the surface firms, cure 24 hours, then label with the actives and their percentages.
Getting an even dose of a potent active into a wax matrix
This is the formulation problem that separates a working rub from a dangerous one. Two mechanisms cause uneven dosing, and they have different fixes.
- Solubility. Menthol and camphor must fully dissolve. Anything left at the bottom of the jug settles and ends up concentrated in the last tin poured.
- Weighability. Anything dosed below about 0.5 g per batch cannot be weighed on a 0.1 g scale. Make a 10 percent predilution of capsicum oleoresin in fractionated coconut oil, so 0.12 g of oleoresin becomes 1.2 g of predilution, and subtract that 1.08 g of carrier from the fractionated coconut line so the total still comes to 100.
- Viscosity window. Stir the predilution in at about 55 C, fluid but thickening, so the active cannot migrate before it sets.
- Pour in one pass, then test one tin before making more: a pea-sized amount on the inner forearm, twenty minutes, then judge. A fingertip dab underestimates a warming rub badly.
The same discipline matters at scale, because the cooling rate changes: see batch scaling and pour temperatures. Why none of this needs a preservative, despite the botanicals, is in anhydrous formulation.
How to adjust it
One change per batch, rebalanced against the infused oil so the total stays at exactly 100.
- Milder cooling: menthol 8 to 5, camphor 3 to 2, adding the 4 points back to the infused oil. This is the version to start with.
- Stronger cooling: menthol 8 to 10. Do not exceed 10 percent at home. Above that the reports are of burning rather than cooling.
- Camphor free: drop camphor and add 1 point to menthol and 2 to the infused oil.
- Milder warming: capsicum oleoresin 0.05 to 0.02, and lean on the ginger infusion instead. Sensible for anyone with thin or reactive skin.
- Warming with no capsaicin: remove the oleoresin, ginger essential oil to 1.0 percent, 0.05 to the fractionated coconut. Safer, and noticeably less effective.
- Firmer for summer: beeswax 11 to 13, or swap 2 points for carnauba, which hardens far more per gram and adds drag.
- Fragrance free: drop the essential oil line into the carrier, which is the right default for daily use, since repeated exposure is how sensitisation happens. Base choices are in carrier oils compared.
Where the regulatory line sits
In the United States a topical product containing menthol at 1.25 to 16 percent, camphor above 3 percent up to 11 percent, methyl salicylate at 10 to 60 percent or capsaicin at 0.025 to 0.25 percent, presented for the relief of muscle or joint pain, is an over-the-counter drug under external analgesic monograph M017. Cite M017, not Part 348. Formula A at 8 percent menthol and 3 percent camphor is squarely inside that. It is not a cosmetic, whatever the label says.
Concentration is only half of it. A claim about pain, aches, strains or arthritis is medicinal on its own, at any concentration, and in the UK and EU the same result arrives through medicinal product by presentation. Make it for your own family and none of this applies. Sell it and all of it does. The cousin product, same actives and different age rules, is the chest rub balm.
- 2 oz (60 mL) tin: 52-56 g fill. 4 oz (120 mL) jar: 105-112 g.
- Keep the batch below 60 C once menthol or camphor is in.
- Predilute anything dosed below 0.5 g per batch, at 10 percent in a bland carrier.
- Batch overage to allow: 8-15 percent.

What to make first
Make the cooling formula at 5 percent menthol, in a 120 g half batch, with no camphor. It will tell you whether your dissolution technique is sound and whether the wax level suits your hands. Only then decide whether you want a warming version, and if you do, buy gloves before you buy the oleoresin. Keep the framing straight: this is a comfort product for ordinary stiffness, and pain that is new, severe or unexplained belongs with a clinician.
Frequently asked questions
Does a muscle rub actually reduce inflammation?
No. A counter-irritant changes what you feel, not what the tissue is doing. Menthol and capsaicin act on sensory nerve endings and produce cold or heat sensations that compete with pain signalling. Nothing measurable happens to swelling or to healing. That is a useful effect for comfort, and it is not an anti-inflammatory one, so do not describe it as such.
How much menthol can I put in a muscle rub?
Eight percent is a strong adult rub and is the level used in this page's cooling formula. US external analgesic monograph M017 recognises menthol from 1.25 to 16 percent as a counterirritant, but the top of that band stings badly on thin skin and offers no extra comfort. Start at 5 percent, test it on yourself, and treat 10 percent as the practical ceiling.
Is arnica salve worth making?
As a traditional preparation with a long history and a pleasant ritual, yes. As a proven analgesic, the evidence is limited and mixed: systematic reviews of topical herbal preparations find the trial base small and of variable quality, with arnica gel among the better studied but far from settled. Use it on unbroken skin only, and do not promise it works.
Why can I not just stir capsicum oleoresin into the pot?
Because the dose is measured in hundredths of a percent and a domestic scale cannot weigh it. A 240 g batch at 0.05 percent needs 0.12 g. Make a 10 percent predilution in fractionated coconut oil, weigh 1.2 g of that instead, and stir it into a batch that is still fluid enough to disperse it evenly.
Can I use a heat pack over a warming balm?
No. External heat plus a counter-irritant is the combination behind most of the serious burn reports involving these products. Regulators have specifically warned about burns from over-the-counter topical muscle and joint rubs, and heating pads, hot baths and occlusive wraps all raise the risk. Apply the balm to bare skin and leave it uncovered.
Why does my cooling balm feel weaker at the bottom of the tin?
Almost always undissolved menthol that settled in the pouring jug before the balm set, or a batch poured over several minutes while it thickened. Stir to a completely clear melt, keep the batch above its setting point until the last tin is filled, and pour in one pass rather than topping tins up later.
Can I make a wintergreen deep-heat rub in the UK or EU?
No. Methyl salicylate is capped at 0.06 percent in leave-on skin products under Annex III entry 324 of Regulation (EC) No 1223/2009, as amended by Regulation (EU) 2022/1531, and must not be used for children under 6. Is a warming rub better than a cooling one?

People have strong preferences, and the same person often prefers cooling for a fresh strain and warming for a stiff, chronic ache. Make both in small batches and let the preference decide.
