A balm for eczema prone skin, and the wording that turns it into a medicine

A high occlusion balm at 6 percent wax for eczema prone skin, in a 100 ml jar poured at 58 to 64 C, with no fragrance, no keratolytics and the regulatory line drawn.

Eczema is a medical diagnosis and its management belongs with a clinician, so this is a formulation page and not a treatment. What follows is a plain occlusive balm designed for one specific brief: hold as much water in the stratum corneum as an anhydrous product can, using the smallest possible number of materials with a known record of causing contact allergy. It also sets out, in detail, the wording that turns the same jar from a cosmetic into a medicine in the UK, the EU and the US.

Short answer

Beeswax 6 percent, refined shea 22 percent, and the rest high-oleic liquid oils and a low-reactivity ester. No fragrance, no essential oil, no botanical extract, no active. Pour a 100 mL jar at 58 to 64 C, cure 48 hours, and take it out with a clean spatula, never fingers. Describe it as an emollient. The moment you say it treats anything, it is a drug.

The brief: maximum occlusion, minimum sensitisation risk

Almost every other formula on this site optimises for feel. This one optimises two variables that pull against each other, and everything in the ingredient list follows from that.

The first is occlusion. Skin that is prone to eczema loses water through the stratum corneum faster than normal skin, in part because of differences in the ceramide profile and lamellar organisation, and an anhydrous film reduces that loss for as long as it is present. That is a real physical effect and it is the entire mechanism on offer here. What it is not is repair: a balm is a blend of triglycerides, and the barrier is built from ceramides, cholesterol and free fatty acids in roughly equimolar proportions. Putting triglycerides on top does not supply the missing classes, as set out under moisturisers and the skin barrier.

The second is sensitisation risk. Compromised skin absorbs more, and presents allergens to the immune system under conditions that favour sensitisation, which is why contact allergy is more common in people with long-standing eczema. Every ingredient you add is another chance to sensitise someone for life. So the formula is short on purpose: six materials with either a long cosmetic record or a low reported reaction rate, and nothing whose only job is to appear on the label.

The formula

Percentages are by weight and total exactly 100. The grams column is for a 100 g batch, which fills one 100 mL jar at about 92 g with 8 g of overage for the jug, the spatula and the thermometer.

Six percent wax is low for a balm and deliberately so. The product has to spread over a forearm in one pass without dragging, and dragging a stiff film across inflamed skin is unpleasant enough that people stop using it. Above about 10 percent wax this blend becomes something you have to work at. Below 6 percent the jar slumps and weeps oil in a warm room.

The choice of sunflower over olive is not arbitrary. A controlled study on adult forearm skin found that repeated application of olive oil damaged the barrier and reduced stratum corneum integrity, while sunflower seed oil did not and modestly improved it, a difference usually attributed to the linoleic acid content and the free fatty acid load. That is the single clearest ingredient-level result in this area, and it points to a high-oleic sunflower grade, which keeps the linoleic benefit in the profile while giving one to two years of oxidative stability rather than six months. The trade-offs across grades are on high-oleic oils.

Note

Beeswax is the one ingredient in this list with a documented allergy of its own, through propolis and pollen residues rather than the wax esters. Reaction rates are low but real, and they are concentrated in people who use bee products or apply balms to broken skin repeatedly. If you or a customer has ever reacted to a beeswax product, use 3 to 4 percent candelilla instead and expect a slightly drier finish. Background is at beeswax and propolis allergy.

Nothing fragranced, nothing botanical

Fragrance materials and their oxidation products are among the most frequently identified contact allergens in Europe, which is why the EU requires named fragrance allergens to be declared individually above 0.001 percent in a leave-on product. Essential oils are fragrance: limonene and linalool form sensitising hydroperoxides on exposure to air, so an oil that was benign when opened is not the same material a year later. On intact skin that is a manageable risk. On fissured, inflamed skin the calculation has no benefit side at all, because no essential oil contributes anything to occlusion. The detail sits at essential oils in balms.

The same reasoning removes botanical extracts and infused oils. Calendula, arnica, chamomile and yarrow are Compositae, whose sesquiterpene lactones are a recognised cause of contact dermatitis, and a home infusion carries an unknown quantity of them with no dose control and no batch-to-batch reproducibility.

Why urea, keratolytics and colloidal oatmeal cannot be in here

These three come up constantly, and they fail for two different reasons that are worth separating.

The physical reason is solubility. Urea is a crystalline solid that dissolves in water, not oil. Stir it into a melted balm and you get a gritty suspension that settles during cooling, feels like sand on inflamed skin, and cannot hydrate anything, because that action depends on being dissolved in the water phase of the stratum corneum. The same applies to lactic acid salts, to glycerin, and to colloidal oatmeal, which is a milled powder that needs water to form the dispersion it works as. Anything water-soluble in an anhydrous formula is covered at solubility in anhydrous formulas.

The regulatory reason is more decisive. Urea below about 10 percent hydrates; above roughly 20 percent it breaks down keratin, which is a pharmacological action, and in several jurisdictions urea products at those levels are handled as medicines. Salicylic acid and colloidal oatmeal are both listed actives in the US over-the-counter skin protectant monograph, so a product containing them at monograph levels and labelled for that use is an OTC drug, with the labelling, facility registration and listing duties that follow. You cannot put a monograph active in a jar, call the jar a cosmetic, and expect the classification to hold. A lower-risk paraffin variant

If the brief is genuinely maximum occlusion at minimum reactivity, plant fats are not the best answer and it would be dishonest to pretend otherwise. White soft paraffin measures the most complete occlusion of any common material, its sensitisation rate is close to the lowest recorded for any topical ingredient, and it does not oxidise into the aldehydes and peroxides an ageing vegetable oil develops. It is unpopular for aesthetic rather than technical reasons.

One catch. Petrolatum at 30 to 100 percent is itself a listed skin protectant active in the US OTC monograph, so this variant sold there with a protectant claim is a drug rather than a cosmetic. Sold with plain moisturising wording it is not: classification follows the claim, not the jar. Material properties are at petrolatum and mineral oil.

Safety

Greasy emollients transfer to clothing, bedding and dressings, and fabric loaded with emollient residue ignites more readily and burns faster and hotter. UK regulators issued a warning after fatal fires, and it applies to paraffin-free products as well as paraffin-based ones. Anyone using a high-occlusion balm generously should keep away from naked flames, cigarettes and gas hobs, and wash affected fabric at a high temperature. This is the single most important safety point on the page and it is not a theoretical one.

Method, the jar, and pouring at 58 to 64 C

  1. Sanitise the equipment. Jug, spatula, thermometer probe and the jar itself, wiped with 70 percent alcohol and allowed to dry fully. There is no preservative in this product and the protection is the absence of water, so nothing damp goes near it. Method at workshop hygiene.
  2. Melt the solids. Beeswax and shea in a water bath at 75 to 80 C (167 to 176 F), held for ten minutes until completely clear. Shea is polymorphic and a partial melt is what produces grain a week later.
  3. Add the liquids. Caprylic/capric triglyceride, squalane and high-oleic sunflower. Return to 75 C (167 F) and stir gently, without whipping air in.
  4. Cool to 58 C (136 F) off the heat, then stir in the tocopherols. There is no fragrance stage in this formula.
  5. Pour at 58 to 64 C (136 to 147 F). A wide jar and only 6 percent wax means the surface sets slowly and dishes as it cools, so pour at the lower end of the band and keep 40 g of the melt back for a top-up ten minutes later. The physics is at pour temperatures.
  6. Cool fast, then cure. Twenty minutes in the fridge, then 48 hours at room temperature before you judge the texture or label it. Wax crystallisation is still finishing for a day or two after the balm looks solid.

A 100 mL jar is chosen because this is a product used generously over large areas, where a 30 mL tin is finished in a week. That size brings the problem dealt with next.

Scoop hygiene on skin that may be broken

An anhydrous balm has a water activity too low for bacteria or fungi to grow, which is why it needs no preservative. That protection is conditional. Every finger dipped into the jar deposits skin flora and, more importantly, traces of moisture; on eczematous skin, which is frequently colonised with Staphylococcus aureus, that is a meaningful inoculum going back into a pot that will be used for months. Enough moisture accumulated in one spot gives micro-organisms somewhere to live, and the result is the fault described at water contamination in balm.

The rule clinical guidance gives for emollient pots is the right one here: take the product out with a clean spoon or spatula, or decant into a smaller pot, and never put fingers back into the main jar. Supply a spatula if you make these for other people, and say on the label why it is there. Preservative-free is not the same as sterile.

Patch testing before you commit a jar

A home use test is not a diagnostic patch test. A clinic applies standardised allergens under occlusion and reads them at 48 and 96 hours; what you can do at home is a repeated open application test, which tells you whether this product reacts on your skin but nothing about which ingredient did it. Do it anyway, because the alternative is finding out over a whole forearm.

Try this

Apply a pea-sized amount to a 2 cm patch of intact, non-flaring skin on the inner forearm or behind the ear. Repeat twice daily for five to seven days and leave it uncovered. You are looking for redness, itch, small bumps or a spreading edge, which typically appear at 24 to 72 hours rather than immediately. The claim line: emollient wording versus treatment wording

The same jar of fat is a cosmetic or a medicine depending on what you say about it. In the EU and UK a cosmetic is defined by its purpose, and claims must meet the common criteria of legal compliance, truthfulness, evidential support, honesty and fairness. A product presented as treating or preventing disease falls under medicines law instead, whatever its composition. In the US the split runs through intended use: a claim to affect the structure or function of the body, or to treat a disease, makes the product a drug under the Federal Food, Drug, and Cosmetic Act.

Two practical consequences. First, the safest description of this formula is what it physically is: a fragrance-free occlusive emollient balm for dry skin. Second, everything counts, not just the label. Product name, social media captions, before-and-after photographs and reposted testimonials all form part of the presentation a regulator assesses. How it sits alongside a prescribed regimen

It sits underneath, and it replaces nothing. A person with eczema may be using a prescribed leave-on emollient, a topical corticosteroid or calcineurin inhibitor during flares, and possibly antimicrobial or systemic treatment. A home-made balm substitutes for none of those, including the prescribed emollient, because a proper emollient regime runs to hundreds of grams a week.

Where a balm fits is on small, thickened areas, hands, feet, knuckles and lips, on skin that is not flaring, over the top of the routine rather than instead of it. The honest limits

This balm reduces water loss from the skin it is sitting on, for as long as it is sitting there, and it does that as well as an anhydrous plant-fat product can. That is the whole claim. It does not reduce inflammation, it does not shorten a flare, it does not restore the ceramide profile, and the published evidence for emollients as a class does not show that any particular formulation outperforms another. The decision rule is narrow: use it on non-flaring, dry, thickened skin, over the top of whatever a clinician has already told you to do, with a spatula, after a week of home use testing, and stop at the first sign of a delayed reaction. If skin is weeping, crusted, spreading, painful or infected, a balm is the wrong intervention and applying an occlusive film over it makes the situation harder to assess.

Frequently asked questions

Can I say my balm is for eczema?

Not safely. In the UK and EU a product presented as treating or preventing a disease is a medicine regardless of what is in it, and in the US a treatment claim makes it a drug. Even "for eczema-prone skin" invites a medicinal reading and is assessed alongside your product name, photographs and social media wording. "Fragrance-free emollient balm for very dry skin" is the defensible description.

Why is there no colloidal oatmeal or urea in this formula?

Both are water-soluble or water-dependent, so in an anhydrous balm they sit as an undissolved powder that settles and feels gritty rather than doing anything. Both are also listed actives in the US over-the-counter skin protectant monograph at their working levels, which would make the product a drug there rather than a cosmetic. They belong in water-containing products.

Is petrolatum better than shea butter for eczema-prone skin?

For pure occlusion, yes. White soft paraffin gives the most complete reduction in water loss measured for any common topical material, and its sensitisation rate is among the lowest recorded. Shea is a good emollient with moderate occlusion. If the goal is maximum occlusion with minimum reactivity, the mineral base wins on the physical measurements, whatever its reputation.

How do I stop a big jar going off or getting contaminated?

Never put fingers in it. Take product out with a clean spatula or spoon, or decant a week's worth into a small pot. An anhydrous balm has no preservative and relies on containing no water, so moisture introduced from skin is the failure route. Keep it below 25 C and away from sunlight, and discard it if it smells stale or looks cloudy.

Should I patch test a balm before using it?

Yes, with a repeated open application test: a pea-sized amount on a 2 cm patch of intact, non-flaring skin, twice daily for five to seven days. Contact allergy typically shows at 24 to 72 hours, not immediately. This tells you whether the product reacts on your skin, not which ingredient caused it, which needs a clinic patch test.

Can this replace the emollient my doctor prescribed?

No. Prescribed emollient regimes involve large quantities used regularly, and prescriptions for corticosteroids or other treatments address inflammation, which no balm does. A home-made balm can go on small, dry, thickened areas on top of an existing routine. If a clinician has given instructions about what to apply and when, those instructions come first.

Why sunflower oil rather than olive oil?

A controlled study on adult forearm skin found repeated olive oil application damaged the barrier and reduced stratum corneum integrity, while sunflower seed oil did not. The difference is usually attributed to the fatty acid profile and free fatty acid content. A high-oleic sunflower grade keeps that advantage while lasting one to two years instead of six months.
